If your insurance has changed please call us immediately. Bring all medications with you to your appointment.
Make sure you get to your appointment 30 minutes early to register and have your current insurance card and I.D.
If you are unable to make your scheduled appointment, it is YOUR responsibility to call our office and reschedule. If you do not notify us within 24 hours of your scheduled appointment time you will be billed $25 for established patients, $75 for new patients and returning patients that have not been seen in our office for over 2 years and up to $300 if you miss a scheduled procedure.
If follow-up visits are necessary, patient must obtain a referral prior to scheduling an office visit.(HMO ONLY)
Do not lose your information. Doing so will result in unnecessary time delays.
Bring all X-rays/MRI films, that pertain to your related GI symptoms. You will not be seen by our specialist without them.
Failure to obtain medical records will result in cancellation of appointment.
